Photo release form
This Photo Use Release Form is entered into and made valid upon signatures from both the Photographer and the Model.

The Model hereby grants and authorizes the Photographer the right to take, edit, alter, copy, exhibit, publish, distribute and make use of any and all pictures or video taken of the Model by the Photographer to be used in and/or for legally promotional materials including, but not limited to, newsletter, flyers, posters, brochures, advertisements, fundraising letter, annual reports, press kits and submissions to journalists, websites, social networking sites and other print and digital communications, without payment or any other consideration.
2.
The authorization extends to all languages, media, formats and markets now known or hereinafter devised.
3.
The authorization shall continue indefinitely, unless the Model otherwise revokes said authorization in writing.
4.
The Model understands and agrees that these materials shall become the property of the Photographer and will not be returned.
5.
The Model hereby holds harmless, and releases the Photographer from all liability, petitions, and causes of action which the Model, their heir, representative, executors, administrators, or any other persons may make while acting on their behalf.
